The sperm donor consent forms are generally signed by the sperm donor as well as the recipient couple. However, there may be some cases, where the sperm donor may be reluctant to sign the form due to some personal reasons. In such a scenario, the couple may request the sperm bank or the fertility clinic to provide them with a list of eligible sperm donors who are willing to sign the consent form.
